Register with the surgery

Registering to be a patient at our surgery is simple and only requires you to complete paper-based forms which you can collect from the surgery you wish to register at. If you’re unable to attend the surgery to collect these forms, please contact the surgery and we can mail these out to you.

Once completed, please return the forms along with a copy of your current medication. You will be asked to attend a New Patient Health Check appointment. This is an important part of the registration process and helps us ensure that we are providing the correct levels of care for any conditions you may have. Following your appointment, your registration will be confirmed.

You will be assigned an accountable GP when you register. You have the right to see any GP of your choosing even if they are not your accountable GP. Please speak to our reception team for more information.

Non-urgent advice: Note

You are able to register at the surgery if you are outside our catchment area, but this can affect your eligibility for home visits should you require them.
